Chain of Custody and Work Order Information

All samples submitted to us for analysis must be accompanied by appropriate documentation. We provide Chain of Custody and Work Order forms without charge. Please contact our Client Services department to obtain Chain of Custody and Work Order forms.

Clients submitting their own documentation must include the following information:

• Client name, address and telephone number


• Project manager to whom the report should be sent


• Project name/number


• Sample identification


• Date and time of collection


• Number and type of container(s)


• Preservation used


• Method of analysis required


• Turnaround time required


• Billing information (P.O. number and/or special requirements)


• Disclosure of known or suspected hazardous substances

• Standard

Our Standard turnaround service is 10 business days.


• Priority

Our Priority turnaround service is 5 business days.


• Rush

Our Rush turnaround service is less than 5 business days, and is available only by prior arrangement with the laboratory. All projects submitted for Rush service require a Rush Authorization Number. To schedule a project for Rush turnaround, please contact our Client Services department.
